Scheduling an Intake Appointment
If you had an IEP, SOP or 504 Plan in high school, have a history of accommodation, or believe you are eligible for accommodations you should register with the Office of Educational Accessibility. A student with a disability should register with the Office of Educational Accessibility to ensure that they are receiving appropriate and consistent accommodations and access to the University and its programs.
To schedule an intake appointment with Educational Accessibility, please contact the Office of Educational Accessibility at 215-717-6818 and ask to make an appointment with Dr. David Thomas. You can also make an appointment online at uartsaccess.youcanbook.me. Appointments are available 10:30 am to 4:00 pm Monday through Friday.
Please fill out the Pre-Intake Questionnaire prior to your appointment.
In order to allow for sufficient time for the thorough review of documentation, all medical or psychiatric records, psychoeducational testing, former IEPs/504 Plans, learning plans, or any other supporting documentation must be received at least one week prior to the scheduled intake appointment. All documentation should be emailed to email@example.com.
Because each student's situation is unique, the Office of Educational Accessibility simply asks that any interested student meet with the Educational Accessibility Advisor. Documentation requirements will vary by situation. Sample documentation guidelines are available below. The Educational Accessibility Advisor will talk to the student and discuss documentation requirements during the intake interview.
Provision of Accommodations
Accommodations are provided on an individualized basis. Depending on the type of accommodations requested, varying levels of documentation may be required. Accommodations must be reasonable in that they do not cause an undue fiscal or administrative burden on the University and that they do not represent a fundamental alteration to the nature of the academic program. Students are expected to meet minimum technical standards, with or without accommodations, in order to register, participate, and earn credit for a course.
** No student should delay meeting with the Office of Educational Accessibility out of concern for not having appropriate paperwork. **
- Documentation should address how the student's disability currently or chronically affects his/her ability to go about the daily routines of independent life in academic and non-academic settings.
- All documentation should include recommendations for accommodations which directly address the student's disability.
